You still need to eat and work out even on steroids. Its not a magic medicine where you inject yourself and you automatically get muscles. You still have to put in the work to get the most out of the roids. You can look at some old school wrestler juiced up and still fat because they wasn't eating right.

It very well have been the fit guy was on roids as well. Roids +heart problems and working out can be more stressful on the heart than a regular person who is fat and out of shape.
