I’m getting lousy performance from my router and want to upgrade it (approx 8 yrs old, asus n-66u dark night).

I think I should probably also relocate it to to somewhere more central.
House is 3 levels including basement, approx 4000sq ft.wired with Ethernet .
Access point, modem and switch are in a corner of the basement, where all the cable drops are.

If I want to put the router upstairs somewhere, don’t I need 1 cable up to the router, then another back down to the switch? Don’t want to do any more wiring.
Is there another way? Is mesh the best solution?

Yes you will need another cable run. You have a few options, booster, mesh, hard wire (this is best imo). You could also move the modem to another location based on if it's coaxial.
